REGISTRATION CHECKLIST

Thank you for registering with Quartz Hill Little League! Please use the checklist
below and included forms to help with the registration process. We look forward
to hosting you for the 2024 season!
Proof of Residency within QHLL Boundaries using School Enrollment Form (included in
this packet) or home address and three residency documents (see Little League
Residency checklist for acceptable forms dated between February 1, 2023 and February
1, 2024 and showing a matching parent from the birth certificate).
Birth Certificate
Medical Release form
Heart Attack Awareness
Concussion Awareness
Volunteer application (for all managers, coaches, and team parents)
Sponsor form (all teams must have a sponsor)
